To be eligible for an ATG start-up credit, Distributors must complete and submit this short form within 45 days of startup. Be advised, you will be asked to provide serial numbers for all VSmarts, probes, sensors and VLLDs in the system at the end of the form. Upon completion, OPW-FMS will review the report and issue a credit to eligible Distributors.
